ORDER FORM

 In addition to direct checkout orders, some items can only be ordered via phone or email. Please click on OPEN ORDER FORM to submit your order inquiry.

Please note the following:

• Only Domestic orders are accepted.
• Sending an email order request does not guarantee you the requested item. 
• If items are in stock, in the requested size, a representative will call your provided phone number to take payment via PayPal.
• Orders are taken on a first come, first served basis.
• In the event that the item is unavailable, you will not receive a response.
• Please make sure to input your email address so that we can send an invoice.
• Payments MUST be made via PayPal must be made within 24 hours.
• PayPal address MUST be confirmed by PayPal or orders will be canceled.

The following information must be included in your email exactly as described. Email requests that don't match this format will be ignored.
